The Carers’ Resource Home from Hospital initiative has achieved a number of ground-breaking achievements in its inaugural year.
A survey reveals the scheme plays a key role in banishing isolation and easing patients back into everyday life, after discharge from hospitals in Airedale and Bradford. It is also driving down readmission rates, offering far-reaching lessons for the wider NHS and saving money for the taxpayer.
The research involving 250 patients has shown only two per cent of those who were supported by the Home from Hospital team were readmitted for the same condition within 30 days.
Other findings included:73.4 per cent of patients felt more confident about coming home; 87.2 per cent said it was good to know there was someone there to help; 89.3 per cent said their visiting team were friendly and helpful; 61.7 per cent were more confident about managing their own health care going forwards and 87.2 per cent would use the scheme again if they needed it.
